I have read the description of activities for the rehearsal camp and understand that there are inherent risks attached to this activity and accept these risks. I also understand that I will repay the Camp for all costs incurred should my child damage Camp Property.

I hereby give my permission for myself and my child(ren) to be photographed, and/or video/audio recorded at rehearsals, performances, and any other OSYO/OSO event throughout the year. I am aware the images and/or recordings may be used as OSO website content, and in print and other media for the purposes of promotion, illustration, advertising, or publication, without compensation. I acknowledge the images and recordings will remain the property of the Okanagan Symphony Society.

Liability Waiver*

The undersigned participant and their parent or legal guardian, if the participant is under the age of 18 years or the parent/legal guardian is involved in volunteer activities, does (do) hereby execute this release, waiver and indemnification for themself/selves, and their heirs, successors, representatives and assigns, and hereby agree(s) and represent(s) as follows: To release the Okanagan Symphony Society, its members, employees, contractors, coaches, conductors, and affiliates from any and all liability, loss, damage, costs, claims and/or causes of action, including but not limited to all bodily injuries and property damage arising out of participating in the OSYO. The undersigned further agree(s) to indemnify the Okanagan Symphony Society, its members, employees, contractors, coaches and conductors, affiliated with the OSYO, and hold them harmless for any liability, loss damage, cost, claim or judgment or settlement which may be brought or entered against them as a result of the undersigned’s participation in said OSYO.
